DIRECTIONS TO S.C.A.L.E.S.

The S.C.A.L.E.S office is located in the Thousand Oaks Surgical Hospital (TOSH). The office entrance is on the Haaland Dr. side of the building (the opposite side of E. Rolling Oaks Dr.).

SAN FERNANDO VALLEY AND ALL POINTS SOUTH/EAST

1. 101 West (towards Ventura )
2. Exit Rancho Road and turn LEFT
3. Pass under Freeway and make first RIGHT onto Haaland Dr. (just passed the park and ride)
4. Continue on road until see Thousand Oaks Surgical Hospital building on Left (we are located in this building)

CAMARILLO , VENTURA, AND ALL POINTS WEST

1. 101 South (towards Los Angeles )
2. Exit Rancho Road and turn RIGHT
3. Make first RIGHT onto Haaland Dr. (just passed the park and ride)
4. Continue on road until see Thousand Oaks Surgical Hospital on Left (we are located in this building)

SIMI VALLEY AND ALL POINTS NORTH

1. 23 South
2. Merge onto 101 North (towards Ventura )
3. Exit Moorpark Road and turn LEFT
4. Turn LEFT onto Rolling Oaks Dr .
5. Continue to end of Rolling Oaks Dr.
6. Thousand Oaks Surgical Hospital is at end (we are located in this building)
7. Proceed around parking lot to front of building for entrance

LOCAL

1. West (towards Camarillo ) on Thousand Oaks Blvd.
2. Turn LEFT onto Moorpark Road (towards 101)
3. Cross under 101
4. Turn LEFT onto Rolling Oaks Dr .
5. Continue to end of Rolling Oaks Dr.
6. Thousand Oaks Surgical Hospital is at end (we are located in this building)
7. Proceed around parking lot to front of building for entrance
